Detailed Claim Form Upload - Instructions

You must complete and submit a Detailed Claim Form so that it is received by February 25, 2019. Your claim may be submitted by mail or online using this website. Claims received after this deadline will be deemed untimely. More information, including how your claim will be assessed, is available in the Court-approved Notice, the FAQs page of this website FAQs, or by calling the Claims Administrator at 1-877-393-1068.

Additionally, Class Members may call Class Counsel at 1-312-431-0888 with any questions about the Claims Resolution Process or Claim Forms.

How to Upload Online:

Before Claim Upload: You will need to download and complete the fillable PDF Detailed Claim Form. To download the form, you will need the Unique ID printed on the Detailed Claim Form you received in the mail. If you did not receive a Claim Form or have lost yours, please contact the Claims Administrator at 1-877-393-1068 for assistance. After downloading the Detailed Claim Form, make sure you complete the form to the best of your ability and that it is signed and dated.

During Claim Upload: You will be able to upload the Detailed Claim Form, the Verification Form, if applicable, and any documentation required of you. Be sure that you have signed and dated the Detailed Claim Form and Verification Form, if applicable, before uploading to this website.

After Claim Filing: After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to them if you have any questions. Please do maintain a copy of your completed Detail Claim Form and any supporting documentation for your records.

How to File by Mail

Fill out, sign, and date the Detailed Claim Form. If applicable, please complete sign, and date the Verification Form. Then, mail your Claim Form and, if applicable, Verification Form to:
